The HIM Specialist employee will be responsible for the following items: 

• Receive releases of information and record on online Release Log.  Process and send information requested within 14 working days.
• Notify/forward subpoenas to HIM Coordinator immediately upon receipt.
• Create new charts for clinicians and physicians.
• Complete transfers/terminations of clients as instructed.
• Perform audits on new and closed charts.
• Review charts for diagnostic entries and/or changes.
• Review charts for current Treatment Plan dates.
• File paperwork in clients’ charts in timely manner.
• Pull and prepare charts as necessary for clinics.
