Job Description

Since the transition between care is an at-risk process for many patients resulting in re-admissions, disengagement from services, arrests, decompensation and other negative outcomes, this position will ensure that any transition in care to or from a facility or program with 24/7 services is successful by using a facilitated hand-off approach.  The position will ensure that barriers to treatment in the transition are addressed and the patient has adequate access to any services needed. The position will coordinate with the Patient Engagement Coordinator to ensure services are not only scheduled but also provided. Additionally, the position will conduct follow up to discharge phone calls to identify any unmet needs and assess satisfaction with services.  

 Duties and Responsibilities:

  • Work with Patient Engagement Coordinator on discharge plans and recommend services and resources for the successful transition of a patient from one program to the next. 
  • Ensure that the planned services and access to resources occur during the transition process.
  • Work through and overcome barriers to treatment as they occur.
  • Touch base with the patient regularly to ensure the patient remains stable and doesn't decompensate due to lack of services or services that are not beneficial.
  • Work with the receiving program or service line toward engagement of the patient.
  • Use motivational interviewing to encourage the patient to follow through with services if needed
  • Serve as a consultant to Outpatient providers if the patient has difficulty within OP services
  • Ensure both the mental health needs and physical health needs of the patient are being addressed. 
  • Work with the patient or their provider around any payer application needs to ensure the patient follows through with the application process
  • Other duties as assigned

Job Requirements

  • Education:  Associates degree, 60 college credits, or HS diploma with 5 years relevant experience
  • Certificate or Professional License: Have or be able to obtain Navigator's license
  • Experience: Prefer two (2) years behavioral health experience
